This anime will immediately be labelled as "Mixed Feelings", because of the fact it is going to be disgusting for some and enjoyable for others. It most certainly is controversial. I think the premise of this show and the overall narrative is a lot more interesting than most people give it credit for, but I can't blame those who refuse to watch it out of principal, as it does indeed include a lot of disgusting behaviours. Those who give this show a terrible rating without ever having watched it though, I cannot agree with. This is unlike any fantasy anime I have ever watched before: The MChere does not start out OP, he doesn't end up with a harem of ladies trying to sleep with him, he isn't the hero - he ends up being enslaved to royalty and his other party members and being treated like actual scum. He is abused repeatedly, while being forced to use his healing ability, which also forces him to relive the pain, suffering and memories of those he heals.
Someone lost a limb by having it sawn-off with a dull blade? Well, he might be able to heal it, but he will relive that whole ordeal as if he himself had had his own leg sawn-off.
He ends up as a shell of a person, only able to bark and beg for his next dose of drugs, before being stamped on like a bug and subject to horrendous sexual acts.

So, what happens to the mind of someone in that position? Well, it breaks. Here, it turns into something dark, wanting to get revenge and inflict every ounce of pain it suffered and more, back on those who tortured it.
That is the main plot of this show and it works.

Is it edgy and cringe at points? Yes.
Does it maybe swerve a bit too close to hentai? Yes, in fact many consider it a hentai that just happened to go "mainstream".
Is it disturbing? Most certainly.

But it also poses a bit of a moral quandary:
"2 wrongs don't make a right", but underdog stories are always popular - so what happens when you have an underdog MC, surrounded by scum, piece-of-shit characters, ending up doing the exact things that were done to him, back to those who wronged him and worse?
Is the MC a villain too, or is he just enacting some juicy revenge?
Is he going too far, or giving just desserts?
Does the fact he doesn't harm those who haven't harmed make him a good person, or is it simply the bare minimum...

Now, you could have that kind of philosophical debate around this anime, but I highly doubt the author regards this as a deep insight into morality. It isn't social commentary or a work of art.
However, it is still a lot better than a lot of generic fantasy trash that gets pumped out every season. Just be prepared to witness things that would make a lot of people wince.
