I've pretty much never found any fellatio scene in any particular manga to be that excitable or noteworthy and most are throwaway precursors to penetration. This portrays fellatio in a deeply emotionally expressive and involved erotic journey that an extremely eager sister and passive-turned-active participate/aggressor brother partake together as they elevate into more erotic acts and their relationship ascends.
